The IT team will contact you within 3 working days upon receiving your submission. For any urgent matter, please whatsapp Choon Tat or James only after you filled up the form.

Type: Select "Request" if you need a new device, select "Troubleshoot" if you are encountering any IT-related problem.

Remarks: Please state the reason of purchase if you selected "Request". Please describe the problem if you selected "Troubleshoot". Please list down the new staff's name if you are requesting a new email address on behalf of staff.
